Swan House is a purpose built care home set over 2 floors.
We cater for Residents living with Dementia, people who require Nursing Care & residential residents.
Our 61-bed nursing and residential care home provides high-quality, person-centred care in a warm, safe and welcoming environment. We are dedicated to supporting older adults with a wide range of needs, including residential, nursing, dementia and end-of-life care, delivered with dignity, respect and compassion at the heart of everything we do.
The home offers 24-hour care from a skilled and committed multidisciplinary team, ensuring that each resident receives tailored support that promotes independence, wellbeing and quality of life. Care homes combine accommodation, personal care and nursing support, alongside daily living assistance such as washing, dressing, meals and medication management .
We pride ourselves on creating a true “home from home,” where kindness, empathy and meaningful relationships underpin our approach. Residents are encouraged to maintain their individuality, make choices about their daily lives and participate in a varied programme of social activities designed to promote engagement, fulfilment and emotional wellbeing .
Our purpose-built facilities include some comfortable en-suite bedrooms, communal lounges, dining areas and accessible outdoor spaces, enabling residents to relax, socialise and enjoy their surroundings. We actively involve families and loved ones, recognising the importance of connection, trust and open communication.
