Tom Wroe, Poppy Walk & Living Well is a specialist residential service supporting up to 36 individuals with Mental Health Disorders, Dementia, Learning Disabilities, Autism, and other complex care needs. Situated in the heart of Meltham village—a vibrant and busy community with a range of local shops and entertainment—we offer a therapeutic, person-centred environment in a stunning Grade II listed building with expansive outdoor space and beautiful views.
We provide tailored support based on individual needs, using care, recovery, or enablement models to empower each resident. Our compassionate and highly trained team brings a wealth of specialist knowledge and experience, enabling us to support complex behavioural needs through a Positive Behaviour Support (PBS) framework.
Our dedicated in-house team—including a Clinical Psychologist, Clinical Lead, and a specialist Therapy Team—works collaboratively to achieve the best possible outcomes for every individual. We embrace and celebrate each person’s uniqueness, ensuring their journey with us is both fulfilling and empowering.
At the heart of our service are our company values and ethos. We invest significantly in our staff—ensuring they receive comprehensive training in areas such as Moving and Handling, PROACT-SCIPr-UK (diversion and distraction), care planning, capacity and community assessments, and social activity support. This investment, combined with our staff’s genuine passion for making a difference, is what makes our team exceptional.
Our Therapy Team, Positive Behaviouralist, and Service Managers develop meaningful, therapeutic daily activities designed to support personal growth, independence, and overall wellbeing. Their collective expertise ensures that residents not only receive high-quality care but also have opportunities to thrive in a supportive and engaging environment.
